Referring Agencies

In order to ensure that we accept clients who most need our assistance, we will only accept new applications by referral.  If you are a church leader, social worker, or school staff member and would like to refer an individual or family for our services, please complete this Referral Form and email it to TheBridgeSouthWindsor@gmail.com or fax it to 860-372-4141.
